Frequently asked questions

What does the Liability Waiver Form collect?

It collects the participant's name and date of birth, a parent or guardian's name for minors, the signer's email, an emergency contact, and optional health notes, then shows the release and captures an electronic signature.

Can participants sign the waiver on their phone before they arrive?

Yes. Share the form link or a QR code when people book or at the front desk. Each signer can download a signed PDF, and you can download any signed waiver from your responses.

Does a liability waiver protect my business in every case?

No waiver covers everything. Many courts will not enforce a release for gross negligence, and rules for minors vary by state and country. Replace the bracketed details with your own and have a lawyer review the wording before you use it.
