Please read this before you sign.
This consent is between [Studio name] (the "Studio") and the client named in this form.
- Age. I am 18 or older, or my parent or guardian is signing with me and has shown ID, as [state] law requires.
- Health. I have answered the health questions truthfully. I am not under the influence of alcohol or drugs.
- Risks. I understand tattooing can cause infection, allergic reaction, scarring and fading, and that results vary with skin type and aftercare.
- Design. I have approved the design, spelling and placement. A tattoo is permanent and removal is costly and may not be complete.
- Aftercare. I will follow the Studio's aftercare instructions and contact a doctor if I see signs of infection.
- Release. I release the Studio and its artists from claims arising from the tattoo, except for their negligence.
- Photos. The Studio may photograph the finished tattoo for its portfolio unless I tell them not to.
By signing below, I confirm that I have read this consent and agree to it.
