Please read this before you sign.

This consent is between [Studio name] (the "Studio") and the client named in this form.

  1. Health. I have listed every medication and skin product above that could affect my skin, and I will tell the Studio about any change before future visits.
  2. Risks. I understand waxing can cause redness, bumps, ingrown hairs, bruising, and in rare cases lifted or broken skin, especially with retinoids or recent sun.
  3. Age. I am 18 or older, or I am the parent or guardian filling in this form for a client under 18, as [state] law requires.
  4. Aftercare. For [number] hours after waxing I will avoid hot baths, saunas, tanning, exercise and fragranced products on the area.
  5. Right to stop. My esthetician may stop or change the service if my skin is not safe to wax.
  6. Release. I release the Studio and its staff from claims arising from the service, except for their negligence.

By signing below, I confirm that I have read this consent and agree to it.

Frequently asked questions

Why does a waxing consent form ask about retinoids?

Retinoids, isotretinoin and recent peels or sunburn thin the skin, so wax can lift it. Asking first lets your esthetician skip or adjust the service.

Can minors get waxed with this form?

Only where your state allows it and with a parent or guardian. Edit the age term to match your local rules and check ID in person.

Do clients sign again at every visit?

Most studios collect it once and ask returning clients to update it when their medications change. You can send the same link again any time.
