This agreement is between [Studio name] (the "Studio") and the participant named in this form.

  1. Risks. Pottery uses wheels, tools, kilns, glazes and clay dust. These can cause cuts, burns, strain and irritation. The participant accepts these risks.
  2. Safety. The participant follows staff instructions, keeps out of the kiln area unless invited and cleans up wet so dust stays low.
  3. Release. The participant releases the Studio and its staff from claims for injury or loss that happen during classes or open studio, except where caused by gross negligence.
  4. Firing. Pieces can crack or change in the kiln. The Studio is not responsible for work damaged during firing.
  5. Unclaimed work. Finished pieces not picked up within [number] days may be recycled or donated.
  6. Minors. A parent or guardian signs for participants under 18.
  7. Cancellations. Classes cancelled less than [number] hours before the start time cost [fee].
