This consent is between [Studio name] (the "Studio") and the client named in this form.
- The service. Extensions are applied one by one to my natural lashes with a medical grade adhesive while my eyes stay closed for about [number] hours.
- Health. I have answered the questions above truthfully and will tell my artist if anything changes before the appointment.
- Risks. I understand lash services can cause eye irritation, redness, swelling or an allergic reaction to the adhesive, even with no history of allergies. The Studio can do a patch test [number] hours before my appointment if I ask.
- Aftercare. I will keep my lashes dry for [number] hours, avoid oil based products, and never pull or pick at the extensions.
- Removal. Extensions are removed only by a trained artist. Natural lashes shed every few weeks, so some extensions will fall out with them.
- Release. I release the Studio and its artists from claims arising from the service, except for their negligence.
- Photos. The Studio may photograph my lashes for its portfolio unless I tell them not to.
By signing below, I confirm that I have read this consent and agree to it.
