Please read this before you sign.
This agreement is between [Business name] (the "Doula") and the client named in this form.
- Fee. The client pays [total fee] for the package above. A retainer of [retainer] reserves the due date, and the rest is due by [number] weeks of pregnancy.
- Included. The package includes [number] prenatal visits, support during labor and birth, and [number] postpartum visits, as chosen above.
- On call. The Doula is on call from [number] weeks until the birth. The client calls when labor starts or if anything changes.
- Backup. If the Doula is sick or at another birth, a trained backup doula attends.
- Scope. The Doula gives emotional and physical comfort and information. The Doula does not perform clinical tasks, give medical advice or make decisions for the client.
- Refunds. If the Doula and backup both miss the birth for reasons in their control, the client receives [refund]. A fast labor or a planned cesarean the Doula is not allowed to attend is not a missed birth.
