Please read this before you sign.

This release is between [Salon name] (the "Groomer") and the dog owner named in this form.

  1. Matting. Matted coats may need to be shaved short for the dog's comfort. Shaving can reveal skin irritation, cuts or sores that were hidden under the mats. Dematting costs [fee].
  2. Health and age. Grooming can be stressful for senior dogs and dogs with health conditions. The owner has shared every condition above and accepts this risk.
  3. Behavior. The Groomer may stop a groom, muzzle the dog, or charge [fee] if the dog is unsafe to handle.
  4. Minor injuries. Even with care, nicks, clipper irritation or a quick cut nail can happen when a dog moves.
  5. Vet care. If the dog needs urgent care, the Groomer may take it to the vet listed above, and the owner pays for that care up to [amount].
  6. Pick up. Dogs collected more than [number] minutes after the pick up time are charged [fee].
  7. Photos. The Groomer may share photos of the finished groom unless the owner opts out.

Frequently asked questions

What should a dog grooming release form cover?

Matted coats, senior and health risks, behavior, minor nicks, emergency vet care, late pick up and photos. This template covers each of them.

Why ask about matting before the groom?

Matted dogs often need a shave down, which can reveal hidden skin problems. Owners agree to that before you start, not after.

Can mobile groomers use it?

Yes. Send the link when you book, and the owner signs on their phone before you arrive.
