This agreement is between [Kennel name] (the "Kennel") and the dog owner named in this form.
- Rates. The owner pays [nightly rate] per night, plus [fee] for medication or extra care, due at pick up.
- Vaccines. The dog is current on rabies, distemper and bordetella, and is free of fleas. Dogs with fleas are treated at the owner's cost.
- Vet care. If the dog seems ill or hurt, the Kennel may take it to the vet listed above or the nearest clinic. The owner pays for that care up to [amount] without further approval.
- Group play. Dogs in group play can get scratches or minor injuries even with supervision. The owner accepts this risk if group play is chosen above.
- Belongings. The Kennel is not responsible for lost or damaged beds, toys or leashes.
- Late pick up. Dogs not collected by [time] on the pick up date are charged another night. A dog left more than [number] days past pick up with no contact may be rehomed under [state] law.
