This agreement is between [Bartending company] (the "Bar Service") and the client named in this form.
- Fee. The client pays [total fee] for [number] bartenders and the service above. A deposit of [deposit] reserves the date and is not refundable.
- Supplies. The party named above supplies the alcohol. The Bar Service brings bar tools and [supplies included]. The client provides ice, cups and mixers unless listed in the package.
- Responsible service. Bartenders check ID, serve only guests 21 or older, and may refuse or stop service to anyone who appears intoxicated. Shots and self serve drinks are not allowed.
- Hours. Service runs between the times above. Extra time is [rate] per bartender per hour.
- Venue and insurance. The client confirms the venue allows alcohol service. The Bar Service carries liquor liability insurance of [amount].
- Cancellation. If the client cancels within [number] days of the event, the deposit is kept.
