This agreement is between [League or club name] (the "League") and the parent or guardian named in this form.
- Fees. The season fee is [amount], due by [date]. Fees are not refunded after [date].
- Risk. Sports involve contact, falls and other risks of injury. I accept these risks for the player.
- Medical care. If the player needs medical attention and I cannot be reached, I authorize the League to arrange first aid or emergency care.
- Conduct. The player and family follow the League's code of conduct and respect coaches, officials and other teams.
- Photos. The League may use team photos on its website and social media unless I ask it not to in writing.
- Release. To the fullest extent the law allows, I release the League, its coaches and volunteers from claims for injury arising from the player's participation, except where caused by gross negligence.
