Frequently asked questions

What does the Employee Self Evaluation Form ask?

The employee's name, role and manager, which review it is for, their proudest wins, a self rating from 1 to 5, goals met and missed, what got in the way, the skill they want to grow, next goals, and the support they need.

When should employees fill in a self evaluation?

About a week before the review meeting. That gives them time to look back properly, and gives the manager time to read every answer before you meet.

Can I use it for probation and quarterly reviews?

Yes. The first question asks which review it is for, with end of year, mid year, quarterly and first 90 days as options. Edit the list to match how your company runs reviews.
