This agreement is between [Studio name] (the "Studio") and the dancer or parent named in this form.
- Tuition. Tuition is [amount] per month, due by the [day] of each month. Missed classes are not refunded.
- Withdrawal. The Studio needs [number] days written notice to stop tuition.
- Recitals. Recital costumes cost [amount] each and are ordered by [date].
- Attendance. Dancers arrive on time in proper dance wear and follow teacher instructions.
- Risk. Dance carries a risk of injury. The dancer takes part at their own risk, and the Studio may arrange first aid if needed.
- Photos. The Studio may use class and recital photos to promote the Studio unless the parent asks it not to in writing.
