This agreement is between [Church name] (the "Church") and the person or group named in this form (the "User").
- Use. The User may use only the spaces and times approved by the Church, including setup and cleanup.
- Fees and deposit. The User pays a use fee of [amount] and a refundable damage deposit of [amount], returned within [number] days if the spaces are left as found.
- Conduct. Events follow the Church's building policies. [Alcohol, smoking and decoration rules] apply to every event.
- Cleanup. The User leaves every space clean, returns furniture, and removes all trash and decorations by the end time.
- Insurance and liability. The User carries liability insurance of at least [amount], is responsible for guests and any damage, and holds the Church harmless for claims from the event.
- Cancellation. The User cancels at least [number] days before the event. The Church may cancel if the event conflicts with its mission or the building is needed for church use.
