This permission is between [Camp name] (the "Camp") and the parent or guardian named in this form.
- Permission. I give permission for my child to take part in all Camp activities for the sessions selected above.
- Health. I have shared all allergies, medications and health needs, and will tell the Camp about any change.
- Medical care. If I cannot be reached in an emergency, the Camp may arrange first aid and medical treatment for my child. I am responsible for any costs.
- Pickup. The Camp will only release my child to me or to the people listed above, and may ask for ID.
- Risks. I understand camp activities carry a risk of injury and I release the Camp from claims arising from them, except for its negligence.
- Photos. The Camp may photograph activities for its own use unless I tell them in writing at [contact email].
