Frequently asked questions

What does a personal trainer intake form ask?

The client's main goal, training experience, whether they want to train in person or online, which days suit them, anything that might limit their training and what success would look like.

When should new clients complete the intake form?

Before the first session. Send it with the booking confirmation so you can plan the session around their goals and schedule.

Does the form replace a health screening?

No. It asks only about anything that might limit training, such as an injury. Use your own health screening if your gym or insurance requires one.
