Signing records the signer's consent, email, IP address, browser and signing time, and each signer can download a signed PDF copy.

## Tutoring agreement

Please read this before you sign.

This agreement is between **[Tutor or business name]** (the "Tutor") and the parent named in this form.

1. **Rate.** Each **[number]** minute session costs **[rate]**, or **[package price]** for a package of **[number]** sessions.
2. **Payment.** Payment is due **[before each session or monthly]** by **[payment method]**.
3. **Cancellations.** Sessions cancelled less than **[number]** hours ahead are charged in full. The Tutor will reschedule any session they cancel.
4. **Late arrivals.** Sessions end at the scheduled time when the student is late.
