Signing records the signer's consent, email, IP address, browser and signing time, and each signer can download a signed PDF copy.

## Participation agreement

Please read this before you sign.

This agreement is between **[League or club name]** (the "League") and the parent or guardian named in this form.

1. **Fees.** The season fee is **[amount]**, due by **[date]**. Fees are not refunded after **[date]**.
2. **Risk.** Sports involve contact, falls and other risks of injury. I accept these risks for the player.
3. **Medical care.** If the player needs medical attention and I cannot be reached, I authorize the League to arrange first aid or emergency care.
4. **Conduct.** The player and family follow the League's code of conduct and respect coaches, officials and other teams.
5. **Photos.** The League may use team photos on its website and social media unless I ask it not to in writing.
6. **Release.** To the fullest extent the law allows, I release the League, its coaches and volunteers from claims for injury arising from the player's participation, except where caused by gross negligence.
