### Does the owner sign over ownership?

Yes. The owner signs a transfer of ownership online, saved as a PDF with an audit trail. It's a sample to start from, not legal advice.

### Can owners fill it in before they bring the pet in?

Yes. Send the link when they call, so your intake team has the full history before the pet arrives.
