## Pest control service agreement

Please read this before you sign.

This agreement is between **[Business name]** (the "Company") and the client named in this form.

1. **Plan and rate.** The Company treats the property on the plan selected above at **[rate]** per visit, with a first visit fee of **[fee]**.
2. **Retreatments.** If covered pests come back between scheduled visits, the Company retreats at no extra charge within **[number]** days of a request.
3. **Prep.** The client follows the prep sheet before each visit, including clearing access to treated areas and moving pet bowls and food.
4. **Products and safety.** The Company uses licensed technicians and applies products by their label. People and pets stay off treated areas for **[number]** hours or as the technician advises.
