Please read this before you sign.

1. **Your history.** The client has shared their full color and chemical history. Past box dye, henna and bleach can change how new color takes.
2. **Patch and strand tests.** The Salon may ask for a patch test **[number]** hours before color, or a strand test on the day. The client may skip it only at their own risk.

### What should a hair color consultation form ask?

Natural and current color, box dye and chemical treatments, the goal, inspiration and current photos, past reactions, scalp sensitivity and how often the client wants to come in.

### Why ask about box dye?

Box dye and henna can react with lightener and change results, so knowing about them helps you plan the right service and timing.

### When should clients fill it out?

Before the appointment, with the booking confirmation. You see the photos and history ahead of time and can book a longer slot for color correction.
