Please read this before you sign.

This agreement is between **[Driving school name]** (the "School") and the student named in this form. A parent or guardian signs for students under 18.

1. **Lessons.** The School gives the lessons in the package chosen above. Each lesson lasts **[number]** minutes in a dual control vehicle.
2. **Fees.** The package costs **[price]** and is due **[when]**. Packages expire **[number]** months after the first lesson.
3. **Permit.** The student carries a valid learner's permit or license to every lesson. Lessons can't go ahead without one.
