## Dog boarding agreement

Please read this before you sign.

This agreement is between **[Kennel name]** (the "Kennel") and the dog owner named in this form.

1. **Rates.** The owner pays **[nightly rate]** per night, plus **[fee]** for medication or extra care, due at pick up.
2. **Vaccines.** The dog is current on rabies, distemper and bordetella, and is free of fleas. Dogs with fleas are treated at the owner's cost.
3. **Vet care.** If the dog seems ill or hurt, the Kennel may take it to the vet listed above or the nearest clinic. The owner pays for that care up to **[amount]** without further approval.
4. **Group play.** Dogs in group play can get scratches or minor injuries even with supervision. The owner accepts this risk if group play is chosen above.
