## Child care agreement

Please read this before you sign.

This agreement is between **[Daycare name]** (the "Provider") and the parent named in this form.

1. **Tuition.** The parent pays **[amount]** a week, due every **[day]**, including days the child is absent.
2. **Hours.** Care runs from **[opening time]** to **[closing time]**. Pickup after closing costs **[late fee]** for each **[number]** minutes.
3. **Illness.** A child with a fever, vomiting or a contagious illness stays home until **[rule]**.
