Signing records the signer's consent, email, IP address, browser and signing time, and each signer can download a signed PDF copy.

## Studio policies

Please read this before you sign.

This agreement is between **[Studio name]** (the "Studio") and the dancer or parent named in this form.

1. **Tuition.** Tuition is **[amount]** per month, due by the **[day]** of each month. Missed classes are not refunded.
2. **Withdrawal.** The Studio needs **[number]** days written notice to stop tuition.
3. **Recitals.** Recital costumes cost **[amount]** each and are ordered by **[date]**.
4. **Attendance.** Dancers arrive on time in proper dance wear and follow teacher instructions.
5. **Risk.** Dance carries a risk of injury. The dancer takes part at their own risk, and the Studio may arrange first aid if needed.
6. **Photos.** The Studio may use class and recital photos to promote the Studio unless the parent asks it not to in writing.
