## Coaching agreement

Please read this before you sign.

This agreement is between **[Coach or business name]** (the "Coach") and the client named in this form.

1. **Sessions.** The package above includes **[number]** sessions of **[length]** each, by **[video, phone or in person]**.
2. **Fees.** The client pays **[amount]** for the package, due **[when]**. Fees are not refundable once the program starts.
3. **Rescheduling.** Sessions moved with less than **[number]** hours notice count as used.
