## Camp permission and release

Please read this before you sign.

This permission is between **[Camp name]** (the "Camp") and the parent or guardian named in this form.

1. **Permission.** I give permission for my child to take part in all Camp activities for the sessions selected above.
2. **Health.** I have shared all allergies, medications and health needs, and will tell the Camp about any change.
3. **Medical care.** If I cannot be reached in an emergency, the Camp may arrange first aid and medical treatment for my child. I am responsible for any costs.
4. **Pickup.** The Camp will only release my child to me or to the people listed above, and may ask for ID.
5. **Risks.** I understand camp activities carry a risk of injury and I release the Camp from claims arising from them, except for its negligence.
6. **Photos.** The Camp may photograph activities for its own use unless I tell them in writing at **[contact email]**.
