Signing records the signer's consent, email, IP address, browser and signing time, and each signer can download a signed PDF copy.

## Repair authorization

Please read this before you sign.

This agreement is between **[Shop name]** (the "Shop") and the customer named in this form.

1. **Diagnosis.** The Shop may inspect and test drive the vehicle to find the problem. Diagnosis costs **[rate]**.
2. **Approved work.** The Shop will only do work up to the amount chosen above. Anything more needs the customer's approval first.
3. **Estimates.** Estimates are based on what the Shop can see. Hidden problems found during the repair will be quoted before work continues.
4. **Parts.** Replaced parts are returned if requested above. Parts under warranty or core exchange may need to go back to the supplier.
5. **Pickup.** Payment is due when the vehicle is picked up. Vehicles left more than **[number]** days after the repair is finished may be charged storage of **[rate]** a day.
